At TICC, Targeted Case Management (TCM) provides personalized, wraparound support to help individuals and families navigate complex systems and access the resources they need to thrive. Our case managers work alongside clients to reduce barriers, strengthen coordination of care, and support overall well-being—both inside and outside the therapy room.

Who Is What Is Targeted Case Management? Case Management For?
Targeted Case Management is a specialized service designed to help individuals with complex needs gain access to medical, social, educational, and community-based supports. Unlike general case management, TCM focuses on targeted populations who may face challenges coordinating services independently.
At TICC, TCM is often used as part of a holistic, trauma-informed care model that supports the whole person—not just one area of life.
Who Is Targeted Case Management For?
Targeted Case Management may be a good fit for:

Eligibility Requirement:
To receive Targeted Case Management services at TICC, clients must be actively enrolled in Medicaid. Medicaid coverage is required in order to be eligible for TCM services.

What Kind of Support Can Targeted Case Management Provide?
Targeted Case Management can support many areas of a client’s life, including:

What Does the Targeted Case Management Process Look Like at TICC?
Getting started with TCM at TICC is simple. It’s as easy as 1, 2, 3:

Referral or inquiry
Clients may contact our office directly or be referred by a TICC provider. -
Intake and assessment
Clients meet with a Targeted Case Management provider to identify needs, goals, and areas of support. -
Ongoing wraparound support
Case managers provide coordination, advocacy, and continued support as client needs evolve.
At TICC, TCM is often offered as a wraparound service to enhance and support other forms of care.
How Targeted Case Management Works Alongside Therapy
Targeted Case Management often complements therapy services at TICC.
Clients receiving both services benefit from collaboration between their therapist and case manager, ensuring support that addresses both mental health needs and real-world challenges. This team-based approach helps promote stability, continuity of care, and holistic wellness.

Medication Management

TICC has partnered with N'Hanse Family Medicine and Esthetics to connect our clients with medication management services that facilitate effective care coordination. Clients interested in medication management can make direct self-referrals to Nurse Practitioner Specialist Leighnia Nance, MSN, APRN, FNP-BC to begin services.

Leighnia Nance, MSN, APRN, FNP-BC
Leighnia T. Nance is a Nurse Practitioner Specialist in Lexington, Kentucky. She graduated with honors in 2015 with her Master's of Nursing. Having more than 17 years of diverse experiences, especially in pediatrics, family medicine, and nurse practitioner, Leighnia T. Nance owns N’Hanse Family Medicine and Esthetics, cooperates with many other doctors and specialists in Baptist Health Medical Group, University of Kentucky, and CHI Saint Joseph Health Hospital. She also works with other local medical facilities to make sure her patients get the best care.
