Targeted Case Management is a primary, direct service provided to adults or children with serious mental illness or emotional disorders who live in the community. TCM is designed to ensure that individuals and their families gain access to needed medical, social, educational services as well as other agencies whose functions are to provide the support, training and assistance required for a stable, safe and healthy community life.
